Symptom Evaluation: A nurse will ask about your symptoms, including chest pain, loss of consciousness, palpitations, tiredness, sitting up during the night, breathlessness, inability to lie down, leg swelling, stomach tightness, nausea, and vomiting.
Lifestyle Recommendations: The team will review your lifestyle and make recommendations on changes, such as diet, exercise, vaccination history, smoking and alcohol consumption habits.
Heart Failure Education: The team will provide you with educational resources to understand your condition and treatment approaches better, including video clips and a heart failure handbook.
Medication Review with a Pharmacist: The pharmacist at the center will review your medications, look for any potential drug interactions, and provide advice about managing your medications at home.
Medical Assessment: An Internal Medicine Specialist in Cardiovascular Disease will assess your signs and symptoms and may recommend additional tests such as ultrasound, Echo, CT-Scan, or CMR.
Rehabilitation and Treatment Plan: The medical team will work with the cardiac rehabilitation team to develop a treatment plan, which could include medication adjustments, monitoring of kidney function, treatment for comorbidities such as diabetes or high blood pressure, and recommendations for exercise.
Discharge Instructions: Before you leave, a pharmacist and a nurse from the Heart Failure Center will provide advice on adjusting, reducing, or stopping medication, initiating new medications, and implementing additional treatments as ordered by the doctor.
